1. Acknowledgment of Risk
I understand that the use of firearms, whether in training or live-fire exercises, is inherently dangerous. I acknowledge that participation in firearms training carries the risk of serious injury, permanent disability, or death. Hazards include, but are not limited to: negligent discharge, ricochets, equipment failure, human error, environmental conditions, and improper handling by myself or others. I voluntarily assume all risks, both known and unknown, associated with this activity, whether caused by negligence or otherwise.
2. Release of Liability
In consideration for being allowed to participate in this training course, I hereby fully and forever release, discharge, indemnify, and hold harmless Fidelity Arms LLC, its owners, instructors, employees, affiliates, officers, volunteers, and representatives from any and all claims, causes of action, demands, liabilities, damages, costs, or expenses, including attorney's fees, arising out of or in connection with my participation in this course. This waiver applies to all injuries, damages, or losses I may suffer, even if caused by the negligence of those released.
3. Medical Certification
I certify that I am in good physical and mental health and capable of safely participating in firearms training. I am not under the influence of any substances that may impair my judgment, including alcohol, drugs, or prescribed medications. I acknowledge that it is my responsibility to inform the instructor of any medical conditions, limitations, or medications that could affect my safety or performance. In the event of a medical emergency, I authorize Fidelity Arms LLC staff to obtain emergency medical treatment for me. I agree to be responsible for any resulting medical expenses.
4. Firearms Safety Rules
I agree to adhere to all firearms safety protocols taught by Fidelity Arms LLC instructors. I understand and agree to abide by the following rules at all times:
• I will treat every firearm as if it is loaded.
• I will never point a firearm at anything I do not intend to destroy.
• I will keep my finger off the trigger until my sights are on the target and I am ready to fire.
• I will be sure of my target and what is beyond it.
• I will comply with all instructor commands and range safety rules without delay.
• I will not handle any firearm unless directed by an instructor.
• I understand that failure to follow safety rules may result in immediate dismissal from the course without refund.
5. Personal Responsibility
I am solely responsible for my actions and the condition of any equipment I bring. I understand that I am responsible for every round I fire. I agree to follow all course procedures and act in a safe, respectful, and responsible manner throughout the training.
6. Media Release
Fidelity Arms LLC may photograph or video portions of the training for documentation, promotional, or educational purposes. I understand that I am not required to be included in media and may opt out.
7. Minors
Participants under the age of 18 must have a parent or legal guardian present and provide consent by signing this document. No minor may participate in firearms training without direct supervision and written approval.
8. Legal Agreement
This agreement shall be governed by the laws of the State of Arizona. If any provision is found to be invalid, the remainder shall remain in full force and effect. I have read this waiver carefully, understand its terms, and sign it voluntarily. No oral representations, statements, or inducements have been made to me to sign this agreement.
